Coronavirus may kill 100,000 to 240,000 in the US despite social distancing measures, say officials

Washington: Top US government scientists battling the coronavirus estimated on Tuesday that the deadly pathogen could kill 100,000 to 240,000 Americans as it ravages the country despite social distancing measures that have closed schools, banned large gatherings, limited travel and forced people to stay in their homes.

Dr Anthony S Fauci, the nation’s leading infectious disease expert, and Dr Deborah L Birx, who is coordinating the coronavirus response, displayed that grim projection at a White House briefing, calling it “our real number” but pledging to do everything possible to reduce it.

As dire as those predictions are, Fauci and Birx said the number of deaths could be much higher if Americans did not follow the strict guidelines vital to keeping the virus from spreading. The White House models they displayed showed that more than 2.2 million people could have died in the United States if nothing were done.

Those conclusions were based on a continuing analysis of cases in the United States and generally matched those from similar models created by public health researchers around the globe. The two public health officials urged people to take the restrictions seriously, and a subdued President Donald Trump, appearing with them, echoed that message, saying that now is not the time to relax.

“I want every American to be prepared for the hard days that lie ahead,” said Trump, who answered questions for more than two hours and predicted that there would be “light at the end of the tunnel,” but warned that “we’re going to go through a very tough two weeks.”

Fauci and Birx showed charts indicating that coronavirus cases in New York and New Jersey have risen far higher than in other parts of the country, a fact that they said gives them hope that the overall number of deaths might be lower if people in the rest of the states follow the guidelines for at least the next month.

But outbreaks in New Orleans, Detroit and other cities are growing quickly, and experts say it is unclear whether social distancing measures can stop them from rising even more in the next few weeks. Recent estimates in Florida suggest that it may be entering a phase of exponential growth.

Fauci, the director of the National Institute of Allergy and Infectious Diseases, stressed that even with those efforts, it is possible that nearly a quarter-million people in the United States could lose their lives.

“As sobering a number as that is, we should be prepared for it,” he said.

Trump displayed none of the carefree dismissiveness that characterised his reaction to the virus in February and early March, when he repeatedly said that “we have it totally under control” and that “it’s going to be just fine.” For most of the briefing on Tuesday, the president appeared to understand the severity of the potentially grave threat to the country.

But asked about whether the impeachment effort had distracted him in the early days of the pandemic, Trump reverted to form, lashing out at Democrats and once again calling it “a phony impeachment” and “a hoax.” He acknowledged that he might have been distracted, but insisted that he still deserved an “A+” for his effort to combat the virus

The president, who on Sunday extended for 30 days the government’s recommendations for slowing the spread of the virus, made it clear that the data compiled by Fauci and Birx convinced him that the death toll would be even higher if the restrictions on work, school, travel and social life were not taken seriously by all Americans.

Trump said the virus was a “great national trial unlike any we have ever faced before,” and said it would require the “full absolute measure of our collective strength, love and devotion” to minimise the number of people infected.

“It’s a matter of life and death, frankly,” he said, offering a sober assessment of the pandemic’s effect on the United States. “It’s a matter of life and death.”

As of Tuesday, more than 183,500 cases of the virus have been reported in the United States, with more than 3,700 deaths — more than 1,500 of which are in New York, the centre of the nation’s outbreak.

Governor Andrew Cuomo, D-NY, told residents of the state that things would continue to get worse — the peak was not expected there for another one to three weeks.

On Wall Street, the S&P 500 declined 1.6 percent on Tuesday, the end of a month in which the index fell 12.5 percent.

The projection released at the White House was the first time that Trump’s administration has officially estimated the breadth of the threat to human life from the disease caused by the coronavirus, COVID-19. In the past several weeks, Birx and Fauci have resisted predicting how many people might die in the pandemic, saying that there was not enough reliable data.

That is no longer the case, they said.

Public health scientists spent the past week constructing a model that could predict how widely the virus would spread in the coming months and how many people who get infected would succumb to the disease. Birx said the result was clear: The only way to minimise deaths is to continue the difficult restrictions on American life.

“There’s no magic bullet. There’s no magic vaccine or therapy. It’s just behaviours,” Birx said. “Each of our behaviours, translating into something that changes the course of this viral pandemic over the next 30 days.”

The new government estimates came to the same conclusion that other researchers have: That even with the isolation efforts already underway to limit the spread of the virus, infections are almost certain to soar, straining the ability of hospitals to care for infected patients and leading to a growing number of deaths.

One of those models, created by scientists at the University of Washington’s Institute for Health Metrics and Evaluation, predicts that deaths from the virus in the United States will rise rapidly during April, reaching a total of about 84,000 by the beginning of August.

The model uses the severe lockdown in Wuhan, China, to calibrate how the outbreak might play out in the United States.

That approach has some critics because control measures imposed in the United States have generally been less stringent than those in Wuhan.

“If we fail at those measures, we face outcomes far worse than any included in the range of possibilities predicted by their model,” said Carl T. Bergstrom, a professor of biology at the University of Washington.

A second study, released on 17 March by the epidemic modelling group at Imperial College London and written by 30 scientists on its coronavirus response team, predicted that if the United States had done nothing to prevent the spread of the virus, 2.2 million people could have died. If, however, the government tried to isolate people suspected of having the virus and people they were in contact with, the number of deaths could be cut in half, the researchers said.

They concluded that only a nationwide suppression effort — an expanded version of efforts now underway across parts of the country — might significantly reduce the death toll. But they warned that such efforts might have to be maintained for long periods to ensure that the threat is over.

Trump, who spent weeks playing down the threat of the virus — and who has retreated from saying that social distancing could be scaled back in mid-April — congratulated himself at the briefing for projections showing that public health measures may significantly limit the national death toll.

“What would have happened if we did nothing? Because there was a group that said, ‘Let’s just ride it out,’” Trump said, without saying what “group” he was referring to. The president noted the estimate that as many as 2.2 million people “would have died if we did nothing, if we just carried on with our life.”

“You would have seen people dying on airplanes; you would have seen people dying in hotel lobbies. You would have seen death all over,” Trump said. By comparison, he said, a potential death toll of 100,000 “is, according to modelling, a very low number.”

Asked whether the projected death toll might be lower still had he called for social distancing measures weeks earlier than mid-March, almost two months after the first confirmed case of the coronavirus in the United States, Trump insisted that he had acted decisively, noting that he had limited air travel from China on 2 February and from Europe six weeks later.

Birx said it was impossible to know whether earlier action would have made a substantial difference until widespread antibody testing revealed how many Americans had already been infected without realising it.

“If there was no virus in the background” in late winter, Fauci added, “there was nothing to mitigate.” If the virus was already widespread at the time, he said, then the answer was “probably yes.”

Trump said that his weeks of largely dismissive commentary about the virus, until a recent abrupt change in tone, were meant to reassure Americans. “I want to be positive. I don’t want to be negative,” the president said. “I want to give people in this country hope.” He also insisted that what “nobody knew about this virus is how contagious it was,” even though health experts were alarmed by its virulence as early as January.

Near the end of the briefing, Trump said that the United States was “going through probably the worst thing the country’s ever seen,” adding that “we lose more here potentially than you lose in world wars as a country.”

Models of the spread of any infectious disease often give widely varying estimates of the ultimate death toll. Modellers use different assumptions on how the disease will spread and how much people change their behaviour to stop new infections.

If you’re not sick with the new coronavirus, should you wear a mask in public? Global health authorities say no. Amid a shortage of masks, the U.S. is sticking with that advice but Tuesday, President Donald Trump suggested people who are worried wear a scarf.

That shortage is so severe that the Joint Commission, which accredits U.S. hospitals, said Tuesday that if facilities can’t provide proper masks, health workers are allowed to bring their own from home.

Front-line health workers have the greatest need for masks. And when people are sick, wearing a mask helps lessen the chances of infecting others. In places where relatives care for the sick at home, the World Health Organization also has recommended they wear a mask.

But “there is no specific evidence to suggest that the wearing of masks by the mass population has any particular benefit,” Dr Mike Ryan, the WHO’s epidemics chief, told reporters Monday.

“In fact, there’s some evidence to suggest the opposite,” he added, noting risks from an improperly fitted mask or touching the face while taking it off or putting it on.

For months as the COVID-19 crisis grew and masks disappeared from store shelves, U.S. health officials have agreed. The virus is believed to spread mostly through droplets from coughs or sneezes, and thus the main advice has been to keep your distance — staying two meters away — in addition to frequent hand-washing and not touching your face. Health workers who may be doing procedures that generate tinier particles are supposed to get high priority for tight-fitting filtering masks.

“Seriously people - STOP BUYING MASKS!” Surgeon General Jerome Adams wrote in a 29 February tweet. “They are not effective in preventing the general public from catching #Coronavirus but if healthcare providers can’t get them to care for sick patients, it puts them and our communities at risk.”

But mask-wearing has long been common in some countries during respiratory outbreaks, especially in parts of Asia. As questions grow about whether people sometimes can spread the virus before realizing they’re sick -- which social distancing is supposed to address — increasingly people ask what it would hurt to wear some form of mask in public.

Trump said Tuesday that his scientific advisers made clear the general public shouldn’t be competing with hospitals and health workers for scarce masks of any type.

His solution: “Use a scarf if you want,” Trump said at the daily White House briefing. “It doesn’t have to be a mask. It’s not a bad idea at least for a period of time.”

Earlier in the day, Dr Anthony Fauci, infectious disease chief at the U.S. National Institutes of Health, had told CNN that once there are enough masks, there might be “some very serious consideration” about broadening the mask recommendations.

For now, the advice posted on the Centers for Disease Control and Prevention’s website: “If you are NOT sick: You do not need to wear a face mask unless you are caring for someone who is sick (and they are not able to wear a face mask).”

Czech billionaire Daniel Kretinsky tests positive for coronavirus; confirmed cases in country cross 2,500-mark

Owner of the Czech energy conglomerate EPH Daniel Kretinsky has tested positive for coronavirus. The business magnate had tested positive on 12 March and has been in self-isolation since then.

As per a report in Bloomberg, Kretinsky said he is running his businesses from isolation and spends 12 hours daily on the phone. He revealed he has no symptoms apart from a slight cold.

Kretinsky has said that even though companies have been hit by the coronavirus pandemic, the blow is not fatal and he will continue to diversify outside the energy sector.

The 44-year-old businessman is the highest-profile Czech citizen to have tested positive for COVID-19.

The Czech Republic has seen over 2,500 cases being recorded and a total of 17 deaths. The coronavirus pandemic has affected over 7 lakh people around the world, killing over 30,000.

According to Bloomberg, Kretinsky’s EPH had 7 billion euros of revenue in 2018. The company’s assets in the Czech Republic, Slovakia, UK, Germany, Hungary, Italy and Poland are valued at around 13.3 billion euros.

Kretinsky has investments in fields other than the energy sector. He is the co-owner and president of Sparta Prague, one of the most popular football clubs of his country. He also has investments in French newspaper Le Monde and German entertainment company ProSiebenSat.1 Media SE, the report added.

Vincent van Gogh's painting stolen from Singer Laren Museum in Netherlands during COVID-19 lockdown

The Hague, Netherlands: A painting by Dutch master Vincent van Gogh was stolen in an overnight smash-and-grab raid on a museum that was closed to prevent the spread of the coronavirus, police and the museum said on 30 March.

The Singer Laren Museum east of Amsterdam said The Parsonage Garden at Nuenen in Spring 1884 by the Dutch master was taken in the early hours of Monday. By early afternoon, all that could be seen from the outside of the museum was a large white panel covering a smashed door in the building's glass facade.

Museum General Director Evert van Os said the institution that houses the collection of American couple William and Anna Singer is “angry, shocked, sad” at the theft.

The value of the work, which was on loan from the Groninger Museum in the northern Dutch city of Groningen, was not immediately known. Van Gogh's paintings, when they rarely come up for sale, fetch millions at auction.

Police are investigating the theft.

“I’m shocked and unbelievably annoyed that this has happened,” said Singer Laren museum director Jan Rudolph de Lorm.

“This beautiful and moving painting by one of our greatest artists stolen - removed from the community," he added. “It is very bad for the Groninger Museum, it is very bad for the Singer, but it is terrible for us all because art exists to be seen and shared by us, the community, to enjoy to draw inspiration from and to draw comfort from, especially in these difficult times.”

The 25-by-57-centimetre (10-by-22-inch) oil on paper painting shows a person standing in a garden surrounded by trees with a church tower in the background.

It dates to a time when the artist had moved back to his family in a rural area of the Netherlands and painted the life he saw there, including his famous work The Potato Eaters, in mostly sombre tones.

Later, he moved to southern France, where he developed a far more colourful, vibrant style of painting as his health declined before his death in 1890.

Police said in a statement that the thief or thieves smashed a glass door to get into the museum. That set off an alarm that sent officers rushing to the museum but by the time they got there the painting and whoever stole it were gone.

A team including forensics and art theft experts was studying video footage and questioning neighbours. Van Os said the museum's security worked “according to protocol,” but he added: “Obviously we can learn from this.”

The Dutch government on 12 March banned large crowds among its measures to halt the spread of the virus, leading several museums to close temporarily.

Before the closure, the museum was hosting an exhibition titled Mirror of the Soul with works by artists ranging from Jan Toorop to Piet Mondrian, in cooperation with Amsterdam’s Rijksmuseum.

The Singer Laren's collection has a focus on modernism such as neo-impressionism, pointillism, expressionism and cubism.

It is not the first high profile theft from the museum. In 2007, thieves stole seven works from its sculpture garden, including a bronze cast of The Thinker by Auguste Rodin. The famous sculpture was recovered a few days later, missing a leg.

New York attorney-general looks into Zoom's privacy practices; app's popularity growing during coronavirus outbreak

Zoom, the videoconferencing app whose traffic has surged during the coronavirus pandemic, is under scrutiny by the office of New York’s attorney-general, Letitia James, for its data privacy and security practices.

On Monday, the office sent Zoom a letter asking what, if any, new security measures the company has put in place to handle increased traffic on its network and to detect hackers, according to a copy reviewed by The New York Times.

While the letter referred to Zoom as “an essential and valuable communications platform,” it outlined several concerns, noting that the company had been slow to address security flaws such as vulnerabilities “that could enable malicious third parties to, among other things, gain surreptitious access to consumer webcams.”

Over the last few weeks, internet trolls have exploited a Zoom screen-sharing feature to hijack meetings and do things like interrupt educational sessions or post white supremacist messages to a webinar on anti-Semitism — a phenomenon called “Zoombombing.”

The New York attorney-general’s office is “concerned that Zoom’s existing security practices might not be sufficient to adapt to the recent and sudden surge in both the volume and sensitivity of data being passed through its network,” the letter said. “While Zoom has remediated specific reported security vulnerabilities, we would like to understand whether Zoom has undertaken a broader review of its security practices.”

With millions of Americans required to shelter at home because of the coronavirus, Zoom video meetings have quickly become a mainstay of communication for companies, public schools and families. Zoom’s cloud-meetings app is currently the most popular free app for iPhones in the United States, according to Sensor Tower, a mobile app market research firm.

Even as the stock market has plummeted, shares of Zoom have more than doubled since the beginning of the year.

As Zoom’s popularity has grown, the app has scrambled to address a series of data privacy and security problems, a reactive approach that has led to complaints from some consumer, privacy and children’s groups.

The company updated its privacy policy Sunday after users reported concerns, and Monday, Eric S Yuan, chief executive and founder of Zoom, posted a link on Twitter to a company blog item about the policy.

In a statement for this article, the company said it took “its users’ privacy, security and trust extremely seriously,” and had been “working around the clock to ensure that hospitals, universities, schools and other businesses across the world can stay connected and operational.”

“We appreciate the New York attorney-general’s engagement on these issues and are happy to provide her with the requested information,” the statement added.

Last week, after an article on news site Motherboard reported that software inside the Zoom iPhone app was sending user data to Facebook, the company said it was removing the tracking software.

As many school districts adopted Zoom to allow teachers to host live lessons with students, some children’s privacy experts and parents said they were particularly concerned about how children’s personal details might be used. Some districts have prohibited educators from using Zoom as a distance-learning platform.

“There is so much we simply don’t know about Zoom’s privacy practices,” said Josh Golin, executive director of the Campaign for a Commercial-Free Childhood, a nonprofit group in Boston.

In the letter, James’ office cited reports that Zoom had shared data with Facebook and asked for further information on “the categories of data that Zoom collects, as well as the purposes and entities to whom Zoom provides consumer data.”

The office expressed concern that the app may be circumventing state requirements protecting student data. To help educators, the company recently expanded meeting limits on free accounts. The attorney general’s office called such efforts “laudable” but also said the company appeared to be trying to offload consent requirements to schools.

The office requested a description of Zoom’s policy for obtaining and verifying consent in primary and secondary schools as well as a description of third parties who received data related to children.

Zoom has said its service for schools complies with federal laws on educational privacy and student privacy.

The letter also asked for details about any changes the company put in place after a security researcher, Jonathan Leitschuh, exposed a flaw allowing hackers to take over Zoom webcams. The letter noted that the company did not address the problem until after the Electronic Privacy Information Centre, a public interest research centre, filed a complaint about Zoom with the Federal Trade Commission last year.

Coronavirus Outbreak: Older people are at risk of dying but they are not the only ones that are vulnerable

Older people remain most at risk of dying as the new coronavirus continues its rampage around the globe, but they’re far from the only ones vulnerable. One of many mysteries: Men seem to be faring worse than women.

And as cases skyrocket in the U.S. and Europe, it’s becoming more clear that how healthy you were before the pandemic began plays a key role in how you fare regardless of how old you are.

The majority of people who get COVID-19 have mild or moderate symptoms. But “majority” doesn’t mean “all,” and that raises an important question: Who should worry most that they’ll be among the seriously ill? While it will be months before scientists have enough data to say for sure who is most at risk and why preliminary numbers from early cases around the world are starting to offer hints.

Not just the old get sick

Senior citizens undoubtedly are the hardest hit by COVID-19. In China, 80 percent of deaths were among people in their 60s or older, and that general trend is playing out elsewhere.

The greying of the population means some countries face particular risk. Italy has the world’s second-oldest population after Japan. While death rates fluctuate wildly early in an outbreak, Italy has reported more than 80 percent of deaths so far were among those 70 or older.

But, “the idea that this is purely a disease that causes death in older people we need to be very, very careful with,” Dr Mike Ryan, the World Health Organization’s emergencies chief, warned.

As much as 10 percent to 15 percent of people under 50 have moderate to severe infection, he said Friday.

Even if they survive, the middle-aged can spend weeks in the hospital. In France, more than half of the first 300 people admitted to intensive care units were under 60.

“Young people are not invincible,” WHO’s Maria Van Kerkhove added, saying more information is needed about the disease in all age groups.

Italy reported that a quarter of its cases so far were among people ages 19 to 50. In Spain, a third are under age 44. In the U.S., the Centers for Disease Control and Prevention’s first snapshot of cases found 29 percent were aged 20 to 44.

Then there’s the puzzle of children, who have made up a small fraction of the world’s case counts to date. But while most appear only mildly ill, in the journal Pediatrics researchers traced 2,100 infected children in China and noted one death, a 14-year-old, and that nearly six percent were seriously ill.

Another question is what role kids have in spreading the virus: “There is an urgent need for further investigation of the role children have in the chain of transmission,” researchers at Canada’s Dalhousie University wrote in The Lancet Infectious Diseases.

Riskiest Health Conditions 

Put aside age: Underlying health plays a big role. In China, 40 percent of people who required critical care had other chronic health problems. And there, deaths were highest among people who had heart disease, diabetes or chronic lung diseases before they got COVID-19.

Preexisting health problems also can increase risk of infection, such as people who have weak immune systems including from cancer treatment.

Other countries now are seeing how pre-pandemic health plays a role, and more such threats are likely to be discovered. Italy reported that of the first nine people younger than 40 who died of COVID-19, seven were confirmed to have “grave pathologies” such as heart disease.

The more health problems, the worse they fare. Italy also reports about half of people who died with COVID-19 had three or more underlying conditions, while just two percent of deaths were in people with no preexisting ailments.

Heart disease is a very broad term, but so far it looks like those most at risk have significant cardiovascular diseases such as congestive heart failure or severely stiffened and clogged arteries, said Dr. Trish Perl, infectious disease chief at UT Southwestern Medical Center.

Any sort of infection tends to make diabetes harder to control, but it’s not clear why diabetics appear to be at particular risk with COVID-19.

Risks in the less healthy may have something to do with how they hold up if their immune systems overreact to the virus. Patients who die often seemed to have been improving after a week or so only to suddenly deteriorate — experiencing organ-damaging inflammation.

As for preexisting lung problems, “this is really happening in people who have less lung capacity,” Perl said, because of diseases such as COPD — chronic obstructive pulmonary disease — or cystic fibrosis.

Asthma also is on the worry list. No one really knows about the risk from very mild asthma, although even routine respiratory infections often leave patients using their inhalers more often and they’ll need monitoring with COVID-19, she said. What about a prior bout of pneumonia? Unless it was severe enough to put you on a ventilator, that alone shouldn’t have caused any significant lingering damage, she said.

Gender Mystery

Perhaps the gender imbalance shouldn’t be a surprise: During previous outbreaks of SARS and MERS — cousins to COVID-19 — scientists noticed men seemed more susceptible than women.

This time around, slightly more than half the COVID-19 deaths in China were among men. Other parts of Asia saw similar numbers. Then Europe, too, spotted what Dr. Deborah Birx, the White House coronavirus coordinator, labelled a concerning trend.

In Italy, where men so far make up 58 percent of infections, male deaths are outpacing female deaths and the increased risk starts at age 50, according to a report from Italy’s COVID-19 surveillance group.

The U.S. CDC hasn’t yet released details. But one report about the first nearly 200 British patients admitted to critical care found about two-thirds were male.

One suspect: Globally, men are more likely to have smoked more heavily and for longer periods than women. The European Center for Disease Prevention and Control is urging research into smoking’s connection to COVID-19.

Hormones may play a role, too. In 2017, University of Iowa researchers infected mice with SARS and, just like had happened in people, males were more likely to die. Estrogen seemed protective — when their ovaries were removed, deaths among female mice jumped, the team reported in the Journal of Immunology.

University develops automatic, hand-held, inexpensive ventilator units to combat coronavirus pandemic

As the demand for ventilators rises with the number of coronavirus patients growing across the globe, a Texas-based university has developed an automatic, hand-held and inexpensive breathing unit that can soon be used to combat the COVID-19 pandemic that has killed over 37,500 people.

A total of 782,365 COVID-19 cases have been reported across more than 175 countries and territories with 37,582 deaths reported so far, according to Johns Hopkins University data.

Across the United States, hospitals are facing shortages of ventilators, some medical device makers, including Philips, have agreed to ramp up supplies.

But because patients diagnosed with or suspected to have COVID-19 often require breathing support, there is widespread concern that these devices won't be developed and shipped quickly enough.

Texas-based Rice University and Canadian global health design firm Metric Technologies have developed an automated bag valve mask ventilation unit that can be built for less than  $300 worth of parts and help patients undergoing treatment for COVID-19.

The collaboration expects to share the plans for the ventilator by making them freely available online to anyone in the world.

The varsity team designed and built a programmable device able to squeeze a bag valve mask. These masks are typically carried by emergency medical personnel to help get air into the lungs of people having difficulty breathing on their own. But the masks are difficult to squeeze by hand for more than a few minutes at a time.

"It's automatic, electric, and works independently of a tech," Wettergreen, a varsity professor and member of the Design Kitchen team, told PTI.

"It's not designed for people who are critical cases, but rather who are in respiratory distress," the professor said.

That delineation is important: the automated Bag Mask Valve (BVM) would take less-critical patients off ventilators and free them up for only those in dire need. The benefit could be a game-changer for those on the front lines of the COVID-19 battle, Wettergreen said.

"When a crisis hits, we use our skills to contribute solutions. If you can help, you should, and I'm proud that we're responding to the call," said the professor.

The design has caught the attention of the Department of Defense, which may authorise the Navy to utilise it in the near future.

It's a huge feat for the small unit, dubbed the Apollo BVM team, whose students worked around the clock and took classes online in order to deliver the project as soon as possible.

Rohith Malya - an assistant professor of emergency medicine at Baylor College of Medicine, an adjunct assistant professor of bioengineering at Rice, and a principal at Metric Technologies - coined the name as a tribute to Rice's history with NASA and former US President John F Kennedy's now-famous speech kicking off the nation's efforts to go to the moon.

"This project appeals to our ingenuity, it's a Rice-based project and it's for all of humanity. And we're on an urgent timescale. We decided to throw it all on the table and see how far we go," he said.

Malya inspired the Rice project two years ago after seeing families try to keep critically ill loved ones at the Kwai River Christian Hospital in Thailand alive by bag-ventilating them for hours on end. He expects the new Apollo BVM to serve that purpose eventually, but the need is now worldwide.

"This is a clinician-informed end-to-end design that repurposes the existing BVM global inventory toward widespread and safe access to mechanical ventilation," Malya said, noting that more than 100 million bag valve masks are manufactured around the world each year.

Coronavirus Outbreak: Donald Trump extends US social distancing guidelines till 30 April as govt expert predicts over 100,000 deaths

Washington: Bracing the nation for a death toll that could exceed 100,000 people, President Donald Trump on Sunday extended restrictive social distancing guidelines through April, bowing to public-health experts who presented him with even more dire projections for the expanding coronavirus pandemic.

It was a stark shift in tone by the president, who only days ago mused about the country reopening in a few weeks. From the Rose Garden, he said his Easter revival hopes had only been “aspirational".

The initial 15-day period of social distancing urged by the federal government expires Monday and Trump had expressed interest in relaxing the national guidelines at least in parts of the country less afflicted by the pandemic. He instead decided to extend them through 30 April, a tacit acknowledgment he'd been too optimistic. Many states and local governments have stiffer controls in place on mobility and gatherings.

Trump's impulse to reopen the country met a sober reality check Sunday from Dr Anthony Fauci, the government's top infectious disease expert, who said the US could experience more than 100,000 deaths and millions of infections from the pandemic. That warning hardened a recognition in Washington that the struggle against the coronavirus will not be resolved quickly even as Trump expressed a longing for normalcy. “I want our life back again,” the president told reporters.

Trump, who has largely avoided talk of potential death and infection rates, cited projection models that said potentially 2.2 million people or more could have died had social distancing measures not been put in place. And he said the country would be doing well if it "can hold" the number of deaths “down to 100,000." He said the best case for the country would be for the death rate to peak in about two weeks.

"It's a horrible number," Trump said, but added, “We all together have done a very good job.”

Brought forward by Trump at the outdoor briefing, Fauci said his projection of a potential 100,000 to 200,000 deaths is “entirely conceivable" if not enough is done to mitigate the crisis. He said that helped shape the extension of the guidelines, which he called “a wise and prudent decision.”

Americans are now being called on to prepare for another 30 days of severe economic and social disruption, as schools and businesses are closed and public life is upended. One in three Americans remain under state or local government orders to stay at home to slow the spread of the virus.

Trump acknowledged that he may be forced to extend the guidelines again at the end of April, but expressed hope that by 1 June, "we should be well on our way to recovery.”

The federal guidelines recommend against group gatherings larger than 10 and urge older people and anyone with existing health problems to stay home. People are urged to work at home when possible and avoid restaurants, bars, non-essential travel and shopping trips.

For more than a week, Trump had been bombarded by calls from outside business leaders who urged him to begin re-opening the nation's economy and warned of catastrophic consequences that could damage his re-election chances if it remained shuttered for much longer.

“The president is right. The cure can't be worse than the disease, and we're going to have to make some difficult trade-offs," Trump's top economic adviser Larry Kudlow had said last Monday, reflecting the thinking of his economic team.

That talk alarmed health experts, who urged Trump to keep encouraging people to stay home. The virus was still spreading, with the peak still weeks away, the experts warned.

In the end, Trump, in the face of dire projections and increasingly alarming images out of New York, sided with his health experts and backed off the idea of loosening recommended restrictions on less impacted parts of the country. "They're the best in the profession and they didn't like that idea,” he said of Fauci and Dr Deborah Birx, head of the White House coronavirus task force.

Trump was clearly moved by the scenes from New York, particularly hard-hit Elmhurst Hospital in his native Queens.

“I've been watching that for the last week on television," he said. ”Body bags all over, in hallways. I've been watching them bring in trailer trucks — freezer trucks, they're freezer trucks, because they can't handle the bodies, there are so many of them. This is essentially in my community, in Queens, Queens, New York," he continued. “I've seen things that I've never seen before.”

Phasing out the recommendations would have been a symbolic nod to business and an affront to public health experts, but may have had little practical impact. States across the country already have their own restrictions in place that, in many cases, are far stricter than the administration's, and those would have remained in place.

Birx and Fauci said even those areas yet to face a significant outbreak must prepare for the eventuality that they will. “This can happen anywhere," Fauci said. "And that's really one of the issues that we're concerned about and why we were so reluctant to pull back at a time when we need to put our foot on the gas as opposed to on the brake.”

The US had more than 139,000 COVID-19 cases reported by Sunday evening, with more than 2,400 deaths. During the course of the Rose Garden briefing, reported deaths grew by several dozen and the number of cases by several thousand.

Most people who contract COVID-19 have mild or moderate symptoms, which can include fever and cough but also milder cases of pneumonia, sometimes requiring hospitalisation. The risk of death is greater for older adults and people with other health problems. Hospitals in the most afflicted areas are straining to handle patients and some are short of critical supplies.

Fauci's prediction would take the death toll well past that of the average seasonal flu. Trump repeatedly cited the flu's comparatively much higher cost in lives in playing down the severity of this pandemic.

Trump's change in tone was previewed Saturday, when the president suggested then backed away from instituting an “enforceable” quarantine of hard-hit New York, Connecticut and New Jersey. Instead, the White House task force recommended a travel advisory for residents of those states to limit non-essential travel to slow the spread of the virus to other parts of the US.

The quarantine notion was strongly opposed by the governors of those states, who argued it would cause panic.

Even as he opted against the quarantine, Trump on Sunday suggested without evidence that hospitals and hospital systems were “hoarding” ventilators and other medical supplies that were needed in other areas of the state. He also encouraged the Food and Drug Administration to streamline approvals for companies seeking to sanitise badly needed respirators so they can be reused.

For weeks, Trump minimised the gravity of the pandemic, and House Speaker Nancy Pelosi on Sunday accused Trump of “denial” in the crisis and called it “deadly."

Asked whether she believes that attitude cost American lives, Pelosi told CNN, “Yes, I am. I'm saying that."

Former Vice President Joe Biden, the likely Democratic presidential nominee, said he wouldn't go so far as to lay the blame for deaths on the president. “I think that's a little too harsh," he told NBC.

लॉकडाउन: पुलिस की क्रूरता की तस्वीरें, सवाल

Coronavirus Outbreak: Donald Trump wants to 'reopen America' by Easter; why that could be a historic mistake

President Donald Trump says he wants the United States “raring to go” in 2 1/2 weeks, on Easter, with “packed churches all over our country.” He and many other political conservatives suggest that we are responding to something like the flu with remedies that may be more devastating than the disease.

We created an interactive model with epidemiologists to show why returning to normal quickly could be a historic mistake that would lead to an explosion of infections, hospitalisations and deaths.

Instead, health experts advise giving current business closures and social distancing a month to slow the pandemic, buying time to roll out mass testing and equip doctors with protective equipment.

Then, depending on where we are, we can think about easing up — while prepared for a new burst of infections that will then require a new clampdown.

“Anyone advising the end of social distancing now needs to fully understand what the country will look like if we do that,” cautioned Dr Tom Inglesby, a health security expert at Johns Hopkins University. “COVID would spread widely, rapidly, terribly, and could kill potentially millions in the year ahead, with huge social and economic impact.”

Social distancing may delay an outbreak more than prevent it, but it is beneficial in two ways.

First, we can use the time to buttress hospitals and test treatments. Second, interventions can flatten the curve and spread infections over a longer time so that the health care system does not become overwhelmed.

Whatever we do, the pain of both disease and business and school closures may be enormous. During the Great Depression, the jobless rate reached 25 percent; today, the president of the Federal Reserve Bank of St. Louis, James Bullard, warns that it might reach 30 percent. People may lose their jobs, savings and homes; they will go without parties, weddings, funerals and graduations that give life connectedness and meaning.

But we know quite a bit about how to ease economic pain: We can send out checks to individuals, and we can incentivize companies to keep workers on the payrolls. Both steps are part of legislation being considered in Washington.

Some prominent conservatives have suggested that it is worth sacrificing some elderly people to keep the economy moving. “I’d rather die than kill the country,” Glenn Beck told viewers.

But that view reflects two fallacies. First, the fundamental force damaging the economy is not the rule book on social distancing but rather an out-of-control virus, and the best way to protect the economy is to rein in the pandemic. Second, while these critics speak nobly about sacrificing themselves, the policies they advocate would more likely lead to the deaths of many other people who are older and in poorer shape.

It is a false choice to say that we must accept the deaths of senior citizens to keep small businesses going. And any easing of social distancing should be based on data.

D Larry Brilliant, an epidemiologist who is a veteran of the eradication of smallpox and now chairman of an organisation called Ending Pandemics, warned that if Trump sends everyone back to work by Easter, “I think history would judge it an error of epic proportions.”

Brilliant said that the entire country probably does not need to be shut down but that we need widespread testing to determine which areas are at risk and which are not. We also desperately need blood testing to determine who has had the disease and is now immune.

If COVID-19 is as fearsome as some believe, our model suggests a grim possibility: It may be that the only way to control it sustainably is with an economic pause too long to be politically sustainable. In that case, we may be headed for a year of alternating periods of easing and tightening economic activity, with the pandemic rising whenever we ease and subsiding whenever we tighten.

Dr David Fisman, a University of Toronto epidemiologist who helped us build this model, suggests that for the next year we may have to tighten social distancing whenever intensive care unit capacity is stretched, and then loosen it when the situation improves. “This gives the economy and the population ‘breaks’ so that people can breathe and businesses can operate,” Fisman said.

There are some hopeful signs that social distancing and business closures can turn the tide, and not just in South Korea and Singapore. In Italy, confirmed new cases have started to drop. And the number of positive test results conducted by the University of Washington, in hard-hit Seattle has stabilised.

Trump seems to be pushing for a relaxation of restrictions partly based on his repeated comparisons of COVID-19 to the flu, warning that “we cannot let the cure be worse than the problem itself.” Meanwhile, Fox News medical correspondent Dr Marc Siegel scoffed this month about the coronavirus: “Worst-case scenario, it could be the flu.”

Modeling the virus underscores why epidemiologists emphasize that this is not the flu and why we should not expect a return to normal within weeks.

One gauge of a virus is how contagious it is. The flu has an infectiousness measure (or R0) of only about 1.5, meaning that each sick person infects on average 1.5 others. In contrast, COVID-19 without social distancing appears to have an R0 of perhaps 2.5.

A second gauge of a virus is how often infected people must be hospitalized. With the seasonal flu that’s roughly 1 percent; with the coronavirus, estimates range from 5 percent to 20 percent.

A higher R0 and higher hospitalization rate conspire to wreak havoc. A single person with the flu can result in the infections of 386 other people over two months, and a handful would be hospitalized. But in that same period one COVID-19 patient could lead to the infections of 99,000 people, of whom nearly 20,000 might need to be hospitalized.

A third measure is lethality, the “case fatality rate,” or percentage of people who contract an illness who eventually die of it. For the flu, this is about 0.1 percent. For COVID-19, there are enormous uncertainties but even in optimal circumstances it may be 10 times greater, roughly 1 percent — although it has been much higher than that in countries like Italy with older populations and overburdened hospitals.

We asked Dr Tara Smith, an epidemiologist at Kent State University, what she would advise a bride and groom whose long-planned wedding is imminent, especially if they can’t afford to reschedule. “They should think about putting other people at risk,” she said, “and what would they think if there was an outbreak because of something happy like a wedding.”

These are not questions we wrestle with each flu season.

We’re living through an extraordinary time, forced to make difficult trade-offs with uncertain information — and one lesson of past pandemics is that early “knowledge” of diseases is often wrong. Yet another lesson of history is the value of responding to pandemics early and firmly. In the 1918 flu pandemic, cities like St. Louis that acted decisively saved many lives. In this crisis, parts of the world like South Korea and Taiwan that acted resolutely have managed far better than those that were more relaxed, like Italy, Iran and the United States.

That’s why epidemiologists and public health experts warn against relaxing too soon. Dr Peter Hotez, a global health expert at Baylor College of Medicine, calls for maintaining controls for at least another month and then reassessing.

Life is about making difficult decisions in the context of uncertainty, and we hope our model will help you appreciate the trade-offs — and the significance of sustained efforts for social distancing not only by the president but also by each of us in our own lives.

US surges past China, Italy in highest COVID-19 cases count with over 85,000 infections, says report

Washington: Over 16,000 confirmed cases were reported in a single day in the US on Thursday as the total number of COVID-19 patients soared to 85,088, the highest for any country, according to data compiled by Worldometer.

The United States surpassed China (81,285) and Italy (80,589) in terms of number of people infected with the novel coronavirus.

According to Worldometers, the website that records confirmed cases of infection and deaths globally, the US by Thursday night had 85,088 individuals infected with coronavirus, of which 16,877 were added just in a single day. A week ago, the number of confirmed cases were 8,000. It has dangerously surged 10 times in the span of a week.

With at least 263 deaths, the US also reported most number of fatalities due to the contagion on a single day on Thursday. At least 1,290 Americans have died due to coronavirus so far. Over 2,000 coronavirus cases were reportedly in a serious condition. The number of confirmed cases as well as the deaths due to COVID-19 are likely to increase substantially in the coming days.

In China, from where it all originated, 3,287 people have died due to the pandemic while Italy has recorded 8,215 such deaths.

US President Donald Trump attributed the spike in the confirmed cases of coronavirus to the large-scale testing of the deadly disease. "I think it's a tribute to our testing," he said, adding "no one knows what the numbers are in China".

Vice President Mike Pence said it is important for hospitals and labs testing the samples to report back to the Centre for Disease Control and Federal Emergency Management Agency so that the government has full visibility to provide the President with the best counsel.

The Abbott Laboratory, he said, submitted to Food and Drug Administration a national request for approval of a point-of-care test. This would be the kind of test where one could go to a doctor and get the test done and have the results in no more than 15 minutes.

According to Dr Deborah Brix, coordinator for the White House Coronavirus Task Force, 55 percent of all new cases continue out of the New York — New York Metro area. This includes New Jersey as well, she said and expressed concern over the coronavirus spread.

"At the same time, 19 of the 50 states that had early cases, but have persistently low level of cases and at this point have less than 200 cases," she said.

In an in interview with CNN, Dr Anthony Fauci, America's top infectious disease expert said it is difficult to tell where the pandemic will go and how long it will last in the US.

This is the relative percentage of asymptomatic infection — and it "influences everything," he said. It influences transmission, contact tracing, and the measures that the authorities need to take. That's why the most important thing to do now is to conduct widespread testing and collect more data, he asserted.

He called for ratcheting up the current level of contact tracing and testing. "We've got to do it better than we are now".

The prestigious Johns Hopkins University, which is also recording all cases of coronavirus, reported 83,836 cases in the United States, ahead of China and Italy.

Dr Brix said so far the US has done 5,50,000 tests. "We are still running somewhere about 14 per cent overall. That means 86 per cent of the people with significant symptoms because remember you had to have a fever and symptoms to get tested at this point. So still 86 per cent are negative," she said.

These are really important facts for the American people, Dr Brix said.

She was critical of some of the modelling studies which had said that there would be 500,000 deaths in the UK and 2.2 million deaths in the US. "They have adjusted that number in the UK to 20,000. Half a million to 20,000. We are looking into this in great detail to understand that adjustment," she said.

"When people start talking about 20 per cent of a population getting infected, it's very scary but we don't have data that matches that based on the experience," Dr Brix said.

She assured Americans that there is no shortage of essential medical supplies including ventilators and ICU beds.
